At the North Toronto Naturopathic Clinic we have a large dispensary of professional natural medicines including botanical tinctures, vitamins, minerals, injectables and homeopathics. We endeavour to provide products that meet our high standards of best possible quality and effectiveness.

Instructions for filling out the reorder form

After downloading the form to your computer, you can fill out the form and send it to us in two ways:

1. Print out the form, fill it out and fax it in OR
2. Fill out the form in the Acrobat Reader application, save it to your computer, and email the form to us as an attachment.

Supplements can still be reordered by phone and picked up 6 days per week during office hours. Alternatively, they can be mailed to you by express post for a small shipping fee. (Note: orders that are sent by mail must be prepaid by Visa or Mastercard)
